Hey everyone, I’m really struggling with stage 3 HS and could use some advice.
I’m a 35-year-old male, and my HS started in my early 20s. I constantly have a fever between 99°F and 101°F (37°C to 38.5°C). I’m on Humira, but it hasn’t worked for me at all. I have pus with a bad smell and ongoing drainage.
I kept a food diary and tried to find my triggers, but I haven’t been successful — my fever keeps fluctuating, and I haven’t noticed any connection to food. I’m also a smoker.

Hello everyone, here is my latest update:
I was hospitalized for about 10 days and received IV antibiotics. After that, my fever and drainage stopped, and I started feeling much better. The infectious disease specialist also told me that antibiotics are only a short-term solution and recommended that I see a surgeon.
About a month later, I found a general surgeon who is now performing deroofing procedures. I am also using Cosentyx as a biologic treatment.
So far, I’ve tried everything I can — IV antibiotics, Cosentyx, and deroofing (except quitting smoking). I’m feeling much better now, although I have significant scarring on my chest because of the deroofing. The deroofing sessions are still ongoing, so we’ll see what the future brings.

Hello idk how long you've been on Humira but I would talk to your dermatologist and go to something else. I started on Humira then Cosyntex,now Bimzelx.. I'd suggest go straight to Bimzelx. Yes I still have flares but they've been more manageable. Definitely what your putting in the your body sets off those triggers stay mindful of that it's hard im guilty of that right now. I was also a smoker of cigarettes many years I quit in August. I do vape tho instead. I do think cutting the actual cigarette smoked has helped as well alot. I'm a stage 3 HS warrior as well. Just had my 3 rd surgery for HS removal within the last 4 years need less to say it been a whirlwind for damn sure. Stay on top of things the best you can don't wait. If need be go to another specialist dermatologist as well. I have 2 dermatologists atp.

If you don’t mind me asking, are you also a stage 3 HS patient?
Do you know what dose of infliximab you’re receiving (for example, the amount based on your weight in pounds or the schedule your doctor put you on)?
Also, did you ever experience constant low-grade fever before switching to infliximab?
I’m trying to understand whether my symptoms are similar to others who improved. Thanks again for taking the time to respond.
Hey it's been a long time I'm just getting back on here.. I hope you are better and you can get the bimzelx. Since then yes now I'm even more better still some issues I'm still working on but Ive made a whole lot of progress. The surgery was large incision and deroofing. I'm still healing since then I'm still in wound care and pain management. You should get involved with that as well if you can it will help also.
