Stage 3 HS (also called Hurley Stage 3) is the most severe form, and it can look quite different from earlier stages. Here's what's typically visible on the skin:

- Large, inflamed lumps resembling boils filled with pus
- Widespread skin discoloration and swelling
- Multiple nodules that have merged into one large affected area
- Yellow areas under the skin where pus has built up
- Lines or tunnels visible under the skin connecting nodules
- Extensive, deep scarring
- Open, draining sores or scabbed-over lesions

The tunnels (called sinus tracts) that form under the skin are a hallmark of stage 3. These can break through to the surface and leak pus or fluid, which can sometimes have a foul smell. Stage 3 most commonly affects areas like the armpits, groin, buttocks, and under the breasts, but can also spread to the neck, waistline, thighs, and perianal area.

As one myHSteam member shared, "I have stage 3 HS. I've had major surgery on both sides of my groin, both armpits, and my butt. I have flare-ups all the time on my waist where my pants rub."

It's worth talking to your doctor about treatment options, as stage 3 can be managed with biologics, oral retinoids, immunosuppressive medications, and surgery.
