I typically wear boxers during the day at home, and 100% cotton underwear when out.

Finding the right underwear when you have HS can make a real difference in comfort and managing flares. The key things to focus on are fabric and fit.
Fabric first:
- Choose natural, breathable materials like 100% cotton, bamboo, flax, or silk
- Avoid synthetic fabrics like polyester or nylon, which trap heat and moisture
- Look for "moisture-wicking" options to keep skin as dry as possible
- Seamless styles can help avoid irritation from seams rubbing on lesions Styles that tend to work well for women:
- Boy shorts or high-cut briefs to reduce friction in the groin and thigh fold
- Loose-fitting options that don't put pressure on the skin
- Boxers (yes, men's boxers!) — many women on myHSteam swear by them
One myHSteam member shared, "I'm female, but I wear men's boxers when I have lesions. Do whatever works for you."
Specialty options worth exploring:
- HidraWear makes underwear specifically designed for HS, with built-in pockets to hold wound dressings in place without tape — and some insurance plans may cover it
- Wearing underwear inside out can help if seamless options are hard to find

Depending where you have them. I use to wear cotton boxers until I got them near buttock area. I hardly wear any underwear, more so,thin bike shorts under clothing. A website called Thigh Society has great options. Only underwear I wear now are seamless. They are worth the cost and comfort! It's like you have nothing on. I usually order from Amazon. I wear them along with a thin pantyliner; the best! At home, I dont wear any underwear, just loose shorts, or biker shorts. God bless you!
