So I've had this ten years now and untill I recently stumbled upon this group I too found it hard to find others that were in the same boat as me. I too get these in my groin and much like you I've had loads of antibiotics from flucloxacillin, clindamicin, clarithramycin and lymcycline (not sure any of these are spelt properly)
I currently take one lymcycline a day and I'm also using driclor you can buy it on Amazon it's a deodorant of sorts which you can use on you're groin.
As for cutting them yourself I totally get it why you'd do this. It's flipping tough going but you can do this

Hello. My name is a Tammi & I’m from Marion, IN. I feel your pain. It has taken almost 2 1/2 yrs to get anyone to listen to me. I have other medical conditions including an LVAD (which is basically an internal heart pump that requires electricity or batteries all the time). So I see Drs all the time. No one wanted to address this issue until I finally got an infectious disease Dr to look at it. He sent me to a dermatologist who didn’t even give me a diagnosis but said they couldn’t help me. Had to finally see a colorectal surgeon who didn’t want to do any surgery because of my other conditions. So I understand about trying all different kinds of things. I finally got them to prescribe Lidocaine gel that I can apply when it’s so sore I can’t handle it. It does give you some relief. I use antibiotic ointment with pain reliever & Benadryl cream on areas that itch. I was on Doxycycline for my another infection that I had & they said it would would help with HS but it doesn’t help me at all. Finally they gave me Bactrim & it helps but doesn’t take care of it. I’m finally scheduled for the surgery on 9/6/23 now after my heart Drs insisted that something be done. This has changed my whole life. I have had breast cancer 3 times (chemo/radiation), CHF (congestive heart failure-caused by my cancer chemo), Diabetes, many other surgeries including a hysterectomy, kidney stone, gall bladder, an infection that almost killed me, a stroke, seizure, then my kidneys were damaged by the treatment for the infection so I had to go on dialysis for about 3 month during which time the lesions really started to get bad; nothing has changed my quality of life as much as this.
I am hoping that the surgery will give me relief & some of my life back.

You might try the lidocaine though. It really helps. And yes I have many scars that have tunneled. Mine is in my groin area which is very hard to deal with.
