This beast has ended any thoughts I had about sex. For real, if they are in the groin/buttocks. Fugedaboutit.

My derm recommends a antibiotic ointment like neosporn or get clindamycin prescribed because the bumps are filled with bacteria so cover lightly with gauze (i wear boxers with guaze on it). I also find a warm compress works really well.

I don't know how everyone else feels, but for me, when the HS-prone spots in my groin are in active outbreak, I don't dare let them get brushed by fabric or bumped- it hurts just to move my legs, so no sex then.
I found the pain is worst when I'd have pustules forming in my groin- stinging, burning.
What I found to help the pain (and force the pustules to come to a head and drain) is DMSO cream- I tap on with finger cots I've cut off vinyl med gloves, then lightly cover with a nonstick pad (monthlies stuck right over the HS spots inside my panties does the trick). That gives the painful pustules a soothing feeling of slight coolness, and helps the skin to gather the bacteria, push it out and heal. The pustules usually drain WITHIN 2 DAYS.
My usual HS skin care is, at shower time, wet the HS spots and apply JĀSÖN TEATREE BODYWASH, let soak in those creases while I wash everywhere else, including my hair. Leave those spots til last for rinsing. DRY THOSE SPOTS THOROUGHLY in front of a clip-on fan in the bathroom after I pat towel-dry.
Then, WHETHER I HAVE AN OUTBREAK OR NOT, morn & night I use SKYORGANICS WITCH HAZEL/TEA TREE FACIAL TONER (yes, it's gentle & I use it on my face too), comes in a spray bottle- either spritz directly on the HS trouble spots or spray on a cotton facial pad and pat gently onto the creases. DRY WITH THE FAN.
That's what helps me keep my skin clear of outbreaks after the DMSO gets them to drain.
I am currently clear, no outbreaks and my skin is near normal skintone.
If you can't find the TT products in your grocery, drug or health food store (skin care aisle) I get them on the big online store that starts with A.
Good luck! 🍀
