My flare ups often occur in my groin area, and I using don’t wear underwear. I don’t want to wear a tampon overnight.

My daughter uses a cervical cup.... She doesn't have HS, I do.... But I haven't had my monthly for 6 yrs.... However I traded in pads for one thing and need different pads for when I sneeze/cough. So I still deal with pad irritation.
However if I still had my period I would definitely try the cups.

How can you manage menstrual flow with HS flares in the groin area while avoiding tight underwear?

Since you typically don't wear underwear due to groin flares, here are some comfortable options for managing your period:

Loose-fitting underwear options:
* Boxer shorts (men's or women's) - Many myHSteam members find these reduce friction while holding pads in place
* Boy shorts - These can minimize rubbing in the groin area
* Mesh postpartum underwear - Lightweight and breathable, designed to hold pads without being tight

Fabric choices matter:
Look for 100% cotton, bamboo, or other breathable materials that won't trap moisture. A myHSteam member shared: "I'm currently wearing stretchy soft bamboo panties from Amazon. I can't even feel them!"

Pad positioning tip:
One member suggested placing a panty liner sideways in the front of underwear. Since pads are designed to absorb without sticking, this prevents fabric from adhering to any wounds.

Consider going a size up in whatever underwear you choose for extra comfort and less pressure on your skin. The goal is to find something loose enough to avoid friction while still holding your pad securely.
